Case management training is essential for preparing professionals to coordinate care, address complex client needs, and improve care quality. Effective case management increasingly relies on strong interdisciplinary collaboration, bringing together professionals from diverse backgrounds to address complex client needs. The aim of this training is to equip professionals with the knowledge, skills and competencies needed to deliver coordinated, person-centered care, especially for individuals with complex health and social needs, from an interdisciplinary perspective.
